Provides direct care as assigned in a participant’s home, contracted facility, or at the PACE Center. Responsible for the collection of data related to participant status; ADL support under the direction of a PACE Homecare Coordinator; ongoing collection and documentation of participant data; assistance with treatments and procedures; and assistance with maintenance of patient care equipment. Assists with a variety of recreation programs and services to meet participant needs for social, physical, and cognitive functioning. Responsible for assisting therapists with skilled therapy and some independent restorative maintenance care of the participants. Essential functions include assisting with eating, bathing, personal grooming and dressing, personal hygiene, applying skin care products, meal planning and preparation, assisting participants in and out of bed, repositioning participants, ambulation, toileting and maintaining continence, laundry duties, shopping, ensuring home safety, running errands, observing vital signs, reminding participants to take prescribed medication, providing escort services and transportation as needed, helping participants with communication, and performing other duties as assigned. This is a non-management job reporting to a supervisor, manager, director or executive. Minimum requirements include a High School Diploma or equivalent, preferred completion of a Nursing Assistant or Patient Care Tech program, and one year experience working with the frail or elderly population. Required certifications include a valid driver’s license, acceptable motor vehicle record, proof of auto insurance, and passing driver training. Basic computer skills and knowledge of office equipment are also required.
