How to Apply During The Database Update
Paper applications will continue to be available and can be obtained at the Smart Start of Forsyth County office through April 3, located at:
7820 North Point Blvd. Winston-Salem, NC 27106
Monday – Friday Â
Hours: 8:00am-1:00pm, 2:00pm-5:00pm
Family Childcare Scholarships
- Parent(s) working a minimum of 25 hours per week,
- Attending school full-time, Or
- A combination of working and attending school for a total of 25 hours per week.
- Forsyth County Resident
Birth certificate or mother’s verification of facts form from hospital
One month of most recent paystubs
Parent’s school schedule showing semester hours, if applicable
If you receive court ordered child support, three-six month print out from child support agency, if applicable
If you receive voluntary child support please provide a letter signed by both parents indicating the amount received monthly.
If you receive SSI, SSA, or SSDI, please provide award letter, if applicable
